The box is a picture of "Operation: Brain Surgery."
The words on the box say "The find and feel matching game." Apparently you reach into his head to find items you need.

I have picture of another box (same position, different kids) that also says in the corner "hee hee that tickles!" which adds the disturbing ness. but the kids didn't have quite the same wild brain-cooping please expression as this one.
